Community Health Needs Assessment

  As of December 31, 2019, Blue Mountain Hospital has completed its Community Health Needs Assessment (CHNA) for the year 2019.

  As part of the Affordable Care Act, not-for-profit hospitals are required to complete a CHNA once every three years. This assessment is used by our staff and board to construct a strategic implementation plan to effectively increase the health of our community. The 2019 report includes a comprehensive assessment of the population Blue Mountain Hospital serves, San Juan County, Utah.

  One of the requirements of the CHNA is to make the report publicly available. This report can be found on Blue Mountain Hospital’s website, bmhutah.org/patients-visitors/community-health-needs-assessment/. A hard copy can also be picked up at BMH’s Administration office.
